Please do not submit your date of birth, insurance information, detailed medical history, symptoms, or other health information through this form — for anything related to your care, call our office directly or use the MyChart patient portal.

Contact form submissions are processed through Netlify, our website hosting provider's form-handling service, before being delivered to our office by email.

How We Use Information

Contact information is used solely to respond to your inquiry and coordinate scheduling. We do not sell, share, or distribute your information to third parties for marketing purposes.

Cookies & Analytics

We use Google Analytics to understand how visitors use this site, but only with your consent. When you first visit, a banner asks whether you'd like to allow analytics cookies. If you accept, Google Analytics sets an anonymous cookie to help us understand usage patterns, such as which pages are visited and how the symptom-search tool is used. Patient Records

All patient medical records and health information are managed through the Columbia Connect / NewYork-Presbyterian MyChart portal, which is governed by HIPAA and Columbia's institutional privacy policies.
