
Total Elbow Replacement
Total elbow replacement replaces the damaged surfaces of the humerus and ulna with linked or unlinked implant components.
When is total elbow replacement considered?
Common indications include inflammatory or rheumatoid arthritis, advanced osteoarthritis or post-traumatic arthritis, severe distal humerus fracture that cannot be reconstructed reliably, painful nonunion or malunion, failed internal fixation, instability with end-stage joint damage, and selected revision situations. Replacement is generally considered after the diagnosis is clear and joint-preserving options are unlikely to provide sufficient pain relief or function.
Evaluation and planning
The examination assesses motion, pain, stability, triceps function, ulnar nerve symptoms, skin and soft-tissue quality, and the ability to perform daily tasks. X-rays show joint destruction, deformity, fracture healing, hardware, and bone loss. CT may be used for complex fracture or revision planning. Prior operative reports, cultures, implant information, and serial imaging are valuable. Infection must be considered after previous surgery, and laboratory testing or aspiration may be required.
The operation
During total elbow replacement, damaged bone and cartilage are removed from the distal humerus and proximal ulna and replaced with implant components. Many modern systems use a linked articulation that provides inherent stability while allowing bending and straightening. Additional procedures may include hardware removal, bone grafting, triceps management, ulnar nerve decompression or transposition, fracture treatment, or reconstruction of bone deficiency. Revision cases can require specialized implants.
Recovery and lifelong activity considerations
The elbow is protected while the incision and soft tissues heal. Motion is restored gradually, followed by functional use. Heavy resisted extension and loading are restricted early. Most surgeons recommend permanent limits on repetitive and heavy lifting to reduce wear, loosening, fracture, and implant failure. Exact guidance is individualized according to implant, bone quality, occupation, and functional needs. Patients who require heavy manual labor or high-load weight training may be better served by another strategy when one is available.
Risks and expectations
Potential risks include infection, wound problems, ulnar nerve injury, stiffness, instability, triceps weakness or failure, fracture, implant wear or loosening, mechanical failure, and revision surgery. Total elbow replacement can be highly valuable for the right patient, but it requires realistic expectations and long-term protection of the reconstruction.
