The Site is not a secure channel for health information and is not a patient portal.
Do not submit symptoms, medical history, images, records, insurance identifiers, or any other health information through this Site or by email to any address listed here. Any information sent this way may not be secure, may not be reviewed, and will not be treated as a clinical communication.
